Juanita Charlene Lewis age 83 of Louisville, KY formerly of Burkesville, KY passed away on Sunday, March 03, 2013 at Jewish Medical Center South. She was born in the Woodburn Community of Warren County, Kentucky to the late Luther and Viola Martinson Chaffin. She was a retired employee of General Electric of Louisville, KY, a member of Burkesville Baptist Church, and a homemaker. In addition to her parents she was preceded in death by her husband, Clarence Lewis, her son, Gearld Wade Lewis, brothers, Earl and Larry Chaffin, sisters, Dolly Woffard, Violet Chaffin, and Evelyn Bunch.

Survivors Include: Her Granddaughter- Renee Charman of Prospect, KY Great Grandchildren- Ryan and Kayla Cundiff Great Great Grandchild- Carly Sister- Bessie Decker of Bowling Green, KY Brothers- Harold Gene and Dale Chaffin of both of Bowling Green, KY and a host of other relatives and friends survive Graveside Service: Thursday, March 07, 2013 at 1:00 P.M. at Grider Memorial Cemetery, Highway 90 W.
